RESUMO

PURPOSE: Iris intraocular implants were developed to manage congenital or traumatic iris defects. However, they are also used to change the color of patient eyes. The aim of this retrospective series was to report complications in patients managed in France after cosmetic implantation. SETTING: Ophthalmological institutions and private ophthalmologists in France. DESIGN: Multicenter retrospective observational study. METHODS: Questionnaires were sent to all ophthalmology departments in university hospitals and to private ophthalmologists. This questionnaire listed demographic and clinical data for each implanted eye with a focus on safety, the description of ocular complications (corneal edema, endothelial cell loss, increased intraocular pressure, and intraocular inflammation), and the therapeutic management implemented. RESULTS: Forty-four questionnaires (87 eyes) were collected, and ultimately, 33 questionnaires (65 eyes) were considered complete and analyzed. Two types of implants were identified. Of the 65 eyes analyzed, only 5 eyes (7.7%) did not experience any complication and 60 eyes (92.3%) had at least 1 complication. The most commonly reported complication was corneal decompensation (78.5%). The diagnosis of glaucoma was made in over half (52.3%) of the cases. Explantation was needed in 81.5% of cases. The mean final visual acuity was 0.45 ± 0.08 logarithm of the minimum angle of resolution (logMAR) (0 to 2 logMAR). CONCLUSIONS: Several ocular complications with a decreased mean visual acuity were described in a young healthy population. In addition, patient information on the safety of this procedure appeared insufficient.

RESUMO

In this paper, we present a new approach for contour detection in the frame of OCT images of the cornea. With the aim of detecting upper and lower curves that define the cornea in radial slices, we have elaborated a three step specific method. The first step consists in finding epithelium and endothelium points (we call them anchor points). Then the image is preprocessed to enhance the contrast and to reduce speckle. In the final step both anchor points are used as starting points for the algorithm: we trace the contour of the cornea pixel by pixel from these two points with a weight criterion. Taking into account geometrical knowledge of the cornea enhances the robustness of the technique. We validate our approach on a bank of scans including low quality images.

Perfect tolerance is expected when one implants a phakic intraocular lens (pIOL) in the anterior segment. Not only should the material be compatible, but the pIOL must respect the anatomy of the anterior chamber. Based on 3 years of experience using an anterior segment optical coherence tomographer (Visante OCT, Carl Zeiss Meditec), I have defined numerous safety criteria for pIOLs. The internal dimensions of the anterior chamber must be considered along different meridians. I propose an objective measurement of the iris dome, the crystalline lens rise (CLR), which is the distance between the anterior pole of the crystalline lens and a line joining the 2 opposite iridocorneal angles. In a series with the Artisan IOL (Ophtec), pigment dispersion syndrome appeared in 70% of cases in which the CLR was greater than 600 microm. Angle-supported IOLs must be placed relative to the anterior chamber's largest diameter; in the same series of cases, the anterior chamber was oval with a large vertical axis in 74% of cases. The posterior face of an angle-supported IOL must have a 700 microm vault to respect the physiological modifications of the crystalline lens. It is difficult to know the posterior chamber's exact diameter as it varies with the horizontal or vertical axis. It also undergoes constant modifications due to accommodation and aging.

PURPOSE: To study the internal diameter of the anterior chamber (AC), which remains a relatively subjective procedure dominated by the white-to-white technique, evaluated with a compass or using anterior segment photographs. In vivo dimensions of the AC were studied with an optical coherence tomographer (AC OCT) prototype. SETTING: Clinique Monticelli, Marseilles, France. METHODS: The dimensions in 107 normal eyes that had never had surgery were studied with the IOLMaster and AC OCT. RESULTS: The mean anterior chamber depth was 3.53 mm +/- 0.35 (SD) with the IOLMaster (Carl Zeiss, Medictec) and 3.64 +/- 0.33 mm with the AC OCT (Carl Zeiss, Medictec). The mean horizontal diameter of the AC was 12.12 +/- 0.44 mm with the IOLMaster and 12.20 +/- 0.44 mm with the AC OCT. With the AC OCT, the anterior chamber's horizontal and vertical diameter could be evaluated; in 74% of the observed eyes, the vertical diameter was larger by at least 100 mum than the horizontal diameter. DISCUSSION: The AC OCT has better reproducibility than the IOLMaster for evaluating the depth or internal diameter of the anterior chamber. This study showed that in 74% of cases, the AC was an oval with a larger vertical axis, which provides evidence of the value of the OCT preoperative examinations for phakic intraocular lens implantation.

PURPOSE: To validate the theory that crystalline lens rise can be used as a safety criterion to prevent pigment dispersion in eyes with an Artisan phakic intraocular lens (IOL) (Ophtec BV). SETTING: Monticelli Clinic, Marseilles, France. METHOD: A comparative analysis of crystalline lens rise in 9 eyes with pigment dispersion and 78 eyes without dispersion was performed. All eyes had previous implantation of an Artisan IOL. Anterior segment imaging was done using an anterior chamber optical coherence tomography (AC OCT) prototype. Crystalline lens rise was defined by the distance between the anterior pole of the crystalline lens and the horizontal plane joining the opposite iridocorneal recesses. RESULTS: The study confirmed that crystalline lens rise can be considered a safety criterion for implantation of Artisan-type phakic IOLs. The higher the crystalline lens rise, the greater the risk for developing pigment dispersion in the area of the pupil. This complication occurred more frequently in hyperopic eyes than in myopic eyes. Results indicate there is little or no risk for pigment dispersion if the rise is less than 600 microm; 67% of eyes with a rise of 600 microm or more developed pupillary pigment dispersion. In some cases in which the IOL was loosely fixated, there was no traction on the iris root and dispersion was prevented or delayed. CONCLUSIONS: Crystalline lens rise should be considered a new safety criterion for Artisan phakic IOL implantation and should also be applied to other types of phakic IOLs. The distance remaining between the crystalline lens rise and a 600 microm theoretical safety level allows one to calculate how long the IOL can safely remain in the eye.

RESUMO

A 59-year-old woman developed progressive, moderate myopia 1 year after routine phacoemulsification and insertion of a soft posterior chamber intraocular lens (IOL). After biomicroscopy, late capsular block was diagnosed and treated with a neodymium:YAG laser posterior capsulotomy. The myopia disappeared immediately. This case was illustrated using optical coherence tomography developed for the anterior segment. After capsulotomy, the IOL moved backward by 448 microm, corresponding to -0.75 diopter of induced myopia.

PURPOSE: To study biometric modifications of the anterior segment with accommodation and age and determine possible applications in areas of anterior segment surgery, particularly implantation of refractive lenses. SETTING: Clinique Monticelli, Marseille, France. METHOD: The study comprised subjects between 7 years of age and 82 years of age in whom anterior chamber biometry was evaluated using 1,310 nm wavelength optical coherence tomography (OCT). The equipment has a fixation target that can be focused and defocused with negative lenses to stimulate natural accommodation. All measurements were performed by the same operator. The horizontal diameter of the AC, the anterior chamber depth (ACD), the horizontal pupil diameter, and the horizontal radius of curvature of the crystalline lens' anterior pole were measured in the unaccommodated state and after stimulating accommodation. RESULTS: Fifty-six subjects (104 eyes) were included; the refractions ranged from +5.0 diopters (D) to -5.0 D. The static and dynamic measurements were compared with ametropia, age, and accommodation. At rest, the mean AC diameter was 12.334 mm, the mean ACD was 3.106 mm, and the mean pupil diameter was 4.258 mm. With 1.0 D of accommodation, the anterior pole moved forward by a mean of 30 microm, the radius of curvature decreased 0.3 mm, and the pupil diameter decreased 0.15 mm. CONCLUSIONS: The AC OCT is a user-friendly instrument for evaluating the anterior segment and examining the AC (cornea, iris, crystalline lens, and iridocorneal angle). The 1,310 nm light wavelength is blocked by pigments, preventing examination behind the iris. However, the AC OCT is capable of good image quality and visualization of the anatomical relationships in the anterior segment, even behind an opaque cornea.

RESUMO

Three phakic intraocular lens (IOL) models were implanted in 3 different patients. With the usual slitlamp examination, it was not possible to determine whether there was contact between the IOLs and the natural crystalline lens. Using the anterior chamber optical coherence tomography (AC OCT) scanner, direct contact between the natural crystalline lens and the 3 phakic IOLs was revealed. A dynamic study of the contact was performed during accommodation. These observations show that examination of the anterior segment with the AC OCT scanner provides new data about the status of the anterior segment after implantation of phakic IOLs.

PURPOSE OF REVIEW: Having solved most of the problems concerning myopia, hyperopia, and astigmatism, it is perfectly understandable that the surgical treatment of presbyopia should be next on the agenda. It is a new challenge where every possible aspect is explored. RECENT FINDINGS: The past five years have been troubled by the debate over von Helmholtz theory on accommodation. Numerous investigations have been carried out on the primate and humans using various procedures. The more we learn about this mechanism, the nearer we will be to finding a solution to presbyopia. It appears essential to refer to recent works confirming von Helmholtz theory. Therefore, understanding presbyopia requires a great deal of optical ingenuity such as monovision, scleral modifications, which still remain controversial, or clear lens exchange or refilling. SUMMARY: In 2003, with all the different techniques available, the surgeon has a wide choice to offer patients that are satisfactory from a practical, theoretical, and ethical point of view. However, these techniques must only be proposed once the patients have been carefully informed.

PURPOSE: To evaluate the efficacy of and specify the conditions required for implantation of an anterior chamber multifocal phakic intraocular lens (IOL) to correct presbyopia. SETTING: Monticelli Clinic, Marseilles, France. METHOD: Fifty-five eyes of 33 patients (21 women, 12 men) had implantation of a foldable anterior chamber multifocal phakic IOL. The initial refraction was between -5.00 diopters (D) and +5.00 D. The IOL had an addition of +2.50 D. An uncorrected distance acuity of 0.6 or better (decimal scale) and an uncorrected near acuity (Parinaud scale) of 3 or better (Parinaud 2 approximately equal to Jaeger 1) was considered a successful postoperative result. RESULTS: The mean follow-up was 42.6 weeks +/- 18 (SD). Postoperatively, the mean refraction was -0.12 +/- 0.51 D, the mean decimal uncorrected distance acuity was 0.78 +/- 0.20, and the mean Parinaud uncorrected near acuity was 2.3 +/- 0.6. Eighty-four percent of eyes achieved an uncorrected distance acuity of 0.60 or better and an uncorrected near acuity of Parinaud 3 or better. The IOL was explanted in 4 eyes for different, but essentially optical, reasons. No significant anatomic complications were observed. CONCLUSIONS: Implantation of an anterior chamber multifocal phakic IOL to correct presbyopia was effective and gave good predictability. The procedure is reversible in cases of patient intolerance to the IOL, unwanted optical phenomena, or complications. Strict inclusion criteria should be used for patient selection.

RESUMO

Anterior segment optical coherence tomography is a new method to explore the anterior chamber. The target can be focused and defocused with positive or negative lenses to reproduce the conditions of natural accommodation. We studied accommodation in an albino patient because the absence of pigment allows the infrared beam to penetrate the iris and explore the modifications of the ciliary body and the crystalline lens during natural accommodation in a human subject.

Introdução: Milhares de ceratoplastias vêm sendo realizadas no mundo inteiro a cada ano. O número de pacientes necessitando de ceratoplastia supera o número de doações de córnea. Na ceratoplastia lamelar a câmara anterior não é penetrada, diminuindo o risco de infecção e hemorragia expulsiva. Além disso, o endotélio corneano é deixado intacto, diminuindo o risco de falência endotelial ou rejeição corneana. O resultado é um período de pós-operatório mais fácil e curto, sem necessidade do uso prolongado de corticóide, evoluindo com melhores resultados refrativos e visuais. Local: Universidade Federal do Rio de Janeiro / Clinique Monticeli. Métodos: Foram estudados 32 olhos de 27 pacientes submetidos ao transplante lamelar com o uso do microcerátomo, o sistema de câmara anterior artificial e botões córneo-esclerais. Estes pacientes foram avaliados considerando sua acuidade visual sem e com correção, o astigmatismo e a ocorrência de complicações. Resultados: A média do tempo de acompanhamento pós-operatório foi de 12,5 +- 6,3 (6-30) meses. A média de idade dos pacientes foi de 51,2 +- 18,6 anos. As indicações cirúrgicas mais freqüentes foram as distrofias corneanas (granular 21,8 porcento; reticular 18,7 porcento; Reis-Bücklers 6,2 porcento). A média da acuidade visual corrigida foi de 0,56 +- 0,19. A média do astigmatismo foi de -2,75 +- 1,75 DC. A freqüência de complicações foi de 31,2 porcento dos casos: 4 casos de invasão epitelial; 2 de infiltrado na interface; 2 de olho seco; 1 de perfuração corneana; e um de reposição do disco. Conclusão: A ceratoplastia lamelar automatizada foi eficaz no tratamento de opacidades corneanas estromais anteriores.

O tratamento da catarata congênita é geralmente motivo de muita discussäo. Estas cataratas impöem, particularmente, problemas ópticos e cirúrgicos devido às suas influências sobre a acuidade visual e das características oculares anatômicas destas crianças. As indicaçöes cirúrgicas näo säo sempre as mesmas. As seguintes técnicas, praticadas pelo mesmo cirurgiäo, säo descritas: extraçäo extracapsular clássica do cristalino,lavagem-aspiraçäo transcorneana, lensectomia via pars plana + ressecçäo do 1/3 anterior de vítreo, facoemulsificaçäo e implante da lente intra-ocular. Alguns exemplos de indicaçäo cirúrgicas säo apresentados. A escolha da técnica cirúrgica influencia diretamente no futuro visual destas crianças, associada à
